Chemotherapy is most often used preoperatively alone or in combination with radiation therapy to treat micrometastases and reduce the tumor size to improve resectability. Cisplatin and 5-fluorouracil the most frequently used agents. Other agents with activity in esophageal cancer include paclitaxel, camptothecin, irinotecan, and vinorelbine. Combination therapy has with response rates between 50% and 70%.

The colon is used if the patient has undergone a partial or total gastrectomy previously or if tumor involves the stomach to preclude a 5-cm margin. The major draw back of using a colon conduit is the need to establish three anastomoses with their complications.
Right sided conduits are based on middle colic artery and have the drawbacks of being short pedicled, thin walled and bulky.
